OUTDOOR FITNESS in WHITMAN: 8:15am-9am Whitman Park (by Playground)

Beginning in April, I will plan my schedule offering 4 class passes, while having 6 days of classes Saturday and/or Sundays.  

*This is SMALL GROUP PERSONAL TRAINING.  Class sizes vary from 3-8 people on average.  We are together to make changes and commit..while picking where your 4 classes will go.  You are welcome to "hit your number" by visiting me in Scituate Wednesdays 9:30am. or dropping into my Zumba class Wednesday nights.  This model worked with HIGH SUCCESS last year.  Very few people missed their package number.  Those who said they would drop in, were very infrequent and not doing as well.....Commitment is KEY to making changes and getting results!! <3

*Payment should be $20 per class in cash ($80 for 4 classes) or $25 per class ($100 for 4) if via Venmo (Diana-Kalivas).

Informed Consent/Liability Waiver:

I understand the potential risks involved in participating in a rigorous physical exercise program.

I assume the responsibility and risks as explained to me. I understand that participating in an exercise program may include, but not be limited to, serious bodily injury, heart attack, stroke, or even death.

I consent voluntarily to participate in an exercise program based on the information provided to me.


I, Certify and advise that, Diana Kalivas, Diana K Fitness LLC, has advised me prior to my commencement of participation in cardiovascular and resistance training programs that such participation could result in physical injury.

That I freely and knowingly assume the risk in such programs, and I hereby waive any right, claim, or cause of action against Diana Kalivas (and any location she teaches ex: Whitman VFW/Dalby Farm in Scituate MA/any venue) and release her and/or her company from any liability for any injury, cost, damage expense or claim, which I or anyone on my behalf might incur as a direct or indirect result of my participation in this cardiovascular and resistance-training program.

That I have read this Liability Waiver form, understand and agree with each of the foregoing points.
